2025-26 Sports Physicals

Sports physical forms will be completed electronically. Use this link to set up your account at Student Central. All student-athletes will register for Richland High School USING THEIR SCHOOL EMAIL. Please create the student athlete account first before setting up the parent accountElectronic forms should be completed and all physical form information should be entered or updated by Monday, November 10th. Winter sports include: JH Boys & Girls Basketball, JH Wrestling, V Boys & Girls Basketball, V Wrestling, V Boys & Girls Swimming, and V Unified Bocce Ball. Failure to meet the deadlines will delay possible participation that day. First time athletes must create a Student Central account and complete all forms electronically. Winter sports athletes: The first day of practice for Winter sports is Friday, November 14th. In order to be eligible for the first day of Winter sports, all physical forms and required information must be completed by Monday, November 10th. Winter sports include: JH Boys & Girls Basketball, JH Wrestling, V Boys & Girls Basketball, V Wrestling, V Boys & Girls Swimming, and V Unified Bocce Ball.
